How to recertify your NREMT EMT
National Registry EMT (NREMT) certification renews on a two-year cycle ending March 31 of your expiration year. Recertification by continuing education requires 40 credits under the National Continued Competency Program (NCCP): 20 national, 10 local/state, and 10 individual (NREMT EMT Recertification; NCCP). The CE application window opens October 1. The recertification-by-CE fee is $25 through September 30, 2026, and $29 for applications submitted October 1, 2026 or later (NREMT). Nationally Registered EMTs renew every two years (NREMT EMT Recertification). You may recertify by continuing education or by the cognitive examination (one pathway is enough).

At least 10% of National Component credits must be pediatric content — ≥2 credits for EMT (NCCP Model 2025). Those pediatric credits count inside the applicable domain totals; they are not an extra bucket on top of 20.
Accepted education (NREMT EMT Recertification): State EMS Office–accepted and CAPCE-accredited education; education from EMS education programs; U.S. accredited academic credit / college courses or credit under the National Registry Alternative Recertification Credits Policy — all must be directly related to EMS patient care. International clinicians must use approved United States education sources (NREMT).
Distributive / online: As of 2022, NREMT states there are no limits on how much online education you can apply on the EMT application (NREMT EMT Recertification).
Does not count (NREMT EMT Recertification): duplicate courses; clinical rotations; instructor courses; management/leadership courses; performance of duty; preceptor hours; serving as a skill examiner; volunteer time with agencies.
Standardized course caps: NCCP Model 2025 lists maximum credits NREMT will accept for named courses (for example CPR-HCP max 4; ACLS max 10; PALS max 12; PHTLS/ITLS max 16). If CAPCE or a state awards more than NREMT’s maximum, only the NREMT maximum applies (NCCP Model 2025 Standardized Course Guide).
Academic credit placement — verify in policy: NREMT publishes an Academic Credit Policy governing college credit conversion and which components academic courses may fill. Treat placement rules as policy-controlled; confirm against the current Academic Credit Policy PDF on nremt.org before advising that a college course fills National vs Local/Individual.
Recertification by examination (alternative)
- Log in, complete a Recertification by Examination application, pay the examination fee ($104 per attempt).
- After 24–48 hours, print the Authorization to Test (ATT) and schedule.
- One attempt between April 1 (one year before current expiration) and March 31 (expiration). Important Dates recommend applying by March 25 to leave scheduling room.
CPR / BLS card (Motion 24-04)
NREMT Certification Scheme materials for EMT were updated for Motion 24-04 — Removal of BLS CPR requirements. The Registry no longer uses a separate CPR card as a Registry eligibility item for certification/recertification under that motion. Your employer or state licensing authority may still require a current CPR card for practice. Store that card separately from your National Registry record if your agency asks for it.
Active vs inactive status
Per NREMT EMT Recertification: Active means you are actively working and providing patient care and requires affiliation with an EMS agency or service (skills verification by Training Officer/Supervisor). Inactive is for clinicians not providing patient care at their level / not affiliated / unable to obtain skills verification; inactive applicants still complete the CE (or exam) requirements and do not need skills verification. Inactive is not a workaround for unfinished education or for license limitations/revocation (NREMT).
Two calendars
NREMT states you should check state license renewal requirements for every state where you are licensed. National Registry expiration and state EMT license expiration are often different dates. Hold them as two credentials in Trust.

After lapse (May 1+): Use NREMT regain pathways — not simple late renewal. Per EMT Re-entry Pathway: if National Registry EMT (and/or applicable state EMT) is lapsed and you are not on the current-state-license path, re-entry generally requires 40 NCCP credits within the past two years, the EMT certification examination, and a State EMS Office–approved BLS skills competency requirement (plus documentation of the lapsed credential when applicable). If you hold a current state EMT license, use the State Licensed Pathway instead (NREMT).
Extensions: Narrow. Per Important Dates, request before current expiration via support@nremt.org with documentation; examples include overseas military deployment or a significant incapacitating personal medical event covering at least two-thirds of the cycle. There is no guarantee of approval (NREMT).
Audits: Retain verification of attendance for education submitted for at least 36 months; failure to produce documentation on audit may result in denial of eligibility to recertify (NREMT EMT Recertification).

- Do I lose time if I renew in October?
- No. After approval, the new expiration is two years after the current expiration date on your account (NREMT EMT Recertification — Recertification Cycles). Early filing does not shorten the next cycle.
- Does renewing NREMT renew my state EMT license?
- No. NREMT encourages you to check each state’s license renewal rules separately (NREMT EMT Recertification). Store National Registry and state EMT as separate credentials.
- What CAPCE courses count?
- CAPCE-accredited education that is directly related to EMS patient care is accepted alongside State EMS Office–accepted education (NREMT). NREMT may cap named standardized courses below the CAPCE award (NCCP Model 2025).
